Your health and safety, and that of our patients, is our top priority. As we approach another flu season, all St. Joseph’s Health employees are required to receive a flu vaccination by Monday, November 15, 2021, unless exempt, as a condition of continued employment.
Starting September 29th, flu vaccines will be available at all three campuses and at Employee Health in Clifton. COVID-19 booster shots will also be available in the vaccine pods after the initial flu vaccine kickoff dates.
Kick off locations:
- SJUMC, Auditorium 2: Wednesday, September 29
10:00 a.m. – 4:00 p.m.
- SJWMC, Conference Room 3AB: Monday, October 4
7:30 a.m. – 4:00 p.m.
After the kickoff dates, both Flu and COVID shots are available in the following locations:
- SJUMC Madison Room, COVID Vaccine, Monday – Friday
7:00 a.m. – 3:30 p.m.
- SJUMC Barclay Room, Flu Vaccine, Monday – Friday
7:00 a.m. – 3:30 p.m.
- SJWMC Employee Health Office, Mondays, Tuesdays and Fridays
COVID and Flu Vaccines
7:30 a.m. – 4:00 p.m.
- SJHRC – On Site for Both Vaccinations, Monday – Friday
- Daily in the Employee Health Office, Clifton, Monday – Friday
8:30 a.m. – 4:30 p.m.
If you receive a flu vaccine outside of our internal Employee Health process, please submit your documentation of Influenza Vaccination to ehdocs@sjhmc.org as soon as you receive the vaccine.
Exemptions
If you have already received an exemption from the Influenza vaccine during the Covid-19 exemption request process, you do not need to request exemption again. If you are unsure which vaccination your exemption was approved for, check your approval letter. If you did not request exemption from the Influenza vaccine as part of your exemption request for Covid-19, you will need to complete either the medical or religious exemption request form below and submit it to vacexemptrequest@sjhmc.org for review. Our panel of medical and pastoral care professionals will review your request and provide a response within 10 calendar days.
